If you would like to receive payment by ACH please complete the information below and include a copy of a voided check or something from the Financial Institution with the routing and account number on it. AUTHORIZATION AGREEMENT FOR ACH

I hereby authorize REACH Media to deposit any amounts owed me, by initiating credit entries to my account at the financial institution (hereinafter “Bank”) indicated on this form. Further, I authorize Bank to accept and to credit any credit entries indicated by REACH Media to my account. In the event that REACH Media deposits funds erroneously into my account, I authorize REACH Media to debit my account for an amount not to exceed the original amount of the erroneous credit. This authorization is to remain in full force and effect until REACH Media and Bank have received written notice from me of its termination in such time and in such manner as to afford REACH Media and Bank reasonable opportunity to act on it

Bank Information

I (we) hereby authorize REACH Media Inc. to initiate deposits to my (our) account indicated below at the depository named below to credit the same to such account.

Document Uploads

This authority will remain in full force and effect until REACH Media has received notification from the undersigned (or either of them, if more than one) in such time and in such manner as to afford Company and Depository a reasonable time to act on it.
